Dear Patients,
I hope this letter finds you well. After much consideration and contemplation, my family and I have made the decision to relocate to Des Moines, Iowa. This decision was not made lightly, and it comes with mixed emotions as I bid farewell to the wonderful community and patients I have had the privilege to serve.
Effective April 2024, I will no longer see patients at Women’s Care. While I am saddened to leave behind the relationships I have built with each of my patients and co-workers, I am also eager for the opportunities and experiences that await us in our new home. Starting May 2024, I will begin seeing patients at UnityPoint Clinic OB/GYN – Lakeview in West Des Moines, Iowa.
I want to express my deepest gratitude to you for entrusting me with your healthcare needs over the past three years. It has been a genuine honor and privilege to serve as your healthcare provider, and I am profoundly grateful for the trust and confidence you placed in me.
Moving forward, I want to ensure a smooth transition for your ongoing healthcare needs. Women’s Care has ten well trained and compassionate providers including eight full spectrum OB/GYN Physicians and two Women’s Health Nurse Practitioners. After practicing closely with each for the past three years, I can personally attest to their abilities and would recommend any of them for your continued obstetric or gynecologic needs. Please contact our scheduling department to reschedule any future appointments with the provider of your preference. Each provider will have access to your medical records via our electronic medical record and I have taken care to document thoroughly in order to facilitate a seamless transition of care. Alternatively, if you would prefer to transfer to a different practice we will transfer records upon request.
If you have any questions or concerns during this transition period, please do not hesitate to reach out directly to Women’s Care. Although I may no longer be your primary healthcare provider, I am committed to ensuring that you receive the support and assistance you need during this time.
Once again, thank you for allowing me to be a part of your healthcare journey. It has been a privilege to serve you and I will always cherish the memories and relationships I formed at Women’s Care. Wishing you continued health and happiness in the years to come.
